How to refer a patient for treatment1

 When patients choose to continue care with Mentavi Health after taking our Diagnostic Evaluation, we make the next steps simple, collaborative, and clinically sound.2

Telemedicine treatment, available in over 40 states

Patients diagnosed by our psychologists through the Mentavi Mental Health Diagnostic Evaluation can continue treatment as warranted by diagnosis with Mentavi Health or bring their results to you for in-person care.

Seamless referral process

To ensure oversight, complete a brief in-person evaluation and referral form to confirm the suitability of any appropriate medications, as warranted by diagnosis.

Personalized, ongoing care

Our clinicians develop individualized treatment plans as warranted by diagnosis and check in regularly: within 7 days for the initial appointment, at 30 days to discuss the treatment plan progress and identify any necessary changes, and every 90 days thereafter to ensure continuity. Should the medication or dosage change, a 30-day check-in will be required.

Comprehensive support

Beyond medication management, when warranted by diagnosis, patients can access therapy and Mental Wellness Coaching to address anxiety, depression, and ADHD.

If your patient does not see the referral email in their inbox within a few minutes, please have them check their Spam or Junk folder for a message from [email protected]

1 Completion of the Mentavi Mental Health Diagnostic Evaluation is required to schedule an appointment with one of our clinicians.

2 Initial and follow-up meetings with providers require separate bookings and are subject to our payment, cancellation, no-show, and refund policies.

Prescriptions, including for controlled substances, are issued only when warranted by diagnosis, clinically appropriate, and in accordance with DEA and State telemedicine regulations.
