If treatment is right for you and available in your state, you can add Medical Treatment or Therapy after you get your results. The Mental Wellness Snapshot is an annual subscription for ongoing check-ins.

Treatment availability varies by state. Mental Wellness Coaching, therapy, and Medical Treatment are available to adults 18+. The Mentavi Mental Health Diagnostic Evaluation is available for ages 6+.

What Medical Treatment visits look like

If Medical Treatment is recommended, care is guided by your diagnosis with ongoing monitoring throughout treatment. Medical Treatment is priced separately from the Diagnostic Evaluation. 

Meet with a clinician — $199 You can meet with a licensed clinician to review your diagnosis and discuss appropriate treatment options.

Check in on progress — $185 About 30 days after your initial visit, you’ll meet with your clinician to review how things are going and make any clinically appropriate adjustments.

Schedule follow-ups — $185 Ongoing follow-up visits are required every 90 days to continue Medical Treatment and monitor your care. A follow-up within 30 days is required if your medication changes.

Complete your evaluation

Take the Mentavi Mental Health Diagnostic Evaluation online. It takes about 60–90 minutes and is reviewed by a doctoral-level psychologist licensed in your state.

Receive your results

Get your results typically within five days after a licensed clinician’s review, along with recommendations for next steps. Timelines may vary.

Choose your path forward

If treatment is clinically appropriate and available in your state, you can review your options and schedule care for an additional cost. Your evaluation is valid for 18 months from your Diagnostic Evaluation or last Medical Treatment visit. Or you can take your results to your preferred clinician.

Insurance and payment options

Your payment options depend on your plan and how you’d like to pay. Here’s what’s available.

Insurance

Mentavi Health doesn’t bill insurance directly or participate in insurance networks. You may be eligible for reimbursement through your plan’s out-of-network benefits, depending on your coverage and deductible. To check, call the member services number on your insurance card and ask about out-of-network reimbursement for the Mentavi Mental Health Diagnostic Evaluation, Medical Treatment, and Therapy.

Mentavi Health provides superbills (itemized statements with billing codes) that you can submit to your insurance provider for reimbursement.

Some insurance providers may require a letter of medical necessity before reimbursing. If your insurer asks for one, contact us and we’ll help with the paperwork.

Mentavi Health doesn’t accept Medicaid, Medicare, or CareCredit.

Out-of-Network Reimbursement Through Mentaya

If you have a PPO or HDHP plan, you may be able to submit out-of-network claims through Mentaya, our independent third-party reimbursement partner, at no out-of-pocket cost to file. Once enrolled, Mentaya automatically files a claim after each appointment, no paperwork required on your end.

Mentaya charges a 5% fee per successfully reimbursed claim, deducted from your reimbursement. If your claim isn’t reimbursed, Mentaya refunds its full fee. Check your eligibility for free, no commitment required; results in about 60 seconds.

Mentaya is an independent third-party service, separate from Mentavi Health. Use of Mentaya’s reimbursement services is optional. Reimbursement is not guaranteed and is subject to your individual plan terms and your insurer’s determination.

HSA and FSA

FSA and HSA funds may be eligible to cover your Diagnostic Evaluation, Medical Treatment, and Therapy. Eligibility depends on your specific plan, so confirm with your plan administrator. Reimbursement applies to services already incurred, and you may need to provide both the original payment receipt and an itemized receipt as supporting documentation.

The Mental Wellness Snapshot is not eligible for FSA/HSA reimbursement.

A note on FSA/HSA cards: card readers sometimes don’t recognize them, and the payment may be declined. If that happens, reach out through Live Chat on the Mentavi Health website and a patient advocate will walk you through it.

Payment Options

You may pay for the Diagnostic Evaluation and treatment appointments in full, or use one of the installment options below.

Diagnostic Evaluation

You may divide the cost into three installment payments:

  • First payment: $73 charged at checkout. A $20 administrative fee is applied to the installment option.
  • Two additional payments: $73 charged on the same date in the following two months
  • Total cost with Pay Plan: $219

Example: If your first payment is made on January 15, the remaining payments will be charged on February 15 and March 15.

Email reminders are sent before each scheduled charge. Access to the assessment remains active during the payment period. Promotional codes cannot be applied to the payment plan option.

Treatment Appointments (Medical Treatment & Therapy)

Klarna is available as a payment option for Medical Treatment and Therapy appointments. Klarna determines approval and payment terms. For questions about how Klarna handles credit checks, contact Klarna directly.

Is treatment included in the cost of the Diagnostic Evaluation?

No. The Diagnostic Evaluation is a separate cost and is required before any clinical service begins. Medical Treatment and Therapy are priced separately.

How long is my Diagnostic Evaluation valid?

Your Diagnostic Evaluation is valid for 18 months from the date of your Diagnostic Evaluation or your last Medical Treatment visit. If that window passes, a new evaluation ($199) is required before you can schedule Medical Treatment.

How often are Medical Treatment appointments, and what do they cost?

Initial appointments are $199. Follow-up appointments are $185 and are required at 30 days after your initial visit, every 90 days after that, and within 30 days of any medication change.

Does the 18-month re-evaluation requirement apply to Therapy?

No. The 18-month window applies to Medical Treatment only. You can schedule Therapy after completing a Diagnostic Evaluation without any re-evaluation requirement.

What is a superbill, and how do I get one?

A superbill is an itemized statement of services that you can submit to your insurance provider for potential reimbursement. Mentavi provides superbills upon request.
